For sponsorship of the 2025 West Virginia Oral Health Summit

For a one-day School-Based Health Conference to be held on June 12, 2025, focused on the delivery of services, encompassing primary care, behavioral health, oral health, and care coordination, across West Virginia

To expand Active Southern West Virginia’s communication systems, recruit new members, and build new partnerships to improve health resource strategies

To expand medically tailored meals and link farm-to-early child care providers to prevent and treat childhood obesity in West Virginia through the Food as Medicine model (over two years)

To serve as a match for Marshall University School of Medicine’s Health Care Rewards to Achieve Improved Outcomes application, an innovative program aimed at reducing overdoses in a 10-county region of West Virginia (over three years)

To improve West Virginia Veterans’ oral health with education, outreach, and data to support system changes and policy development (over two years)

To use curricula adapted by West Virginia University using proven strategies and promising practices, transitioning to the Hope in Action Alliance to ensure institutionalization and sustainability

To build organizational capacity, increase the professionalism of the organization, and complete a strategic plan

To help individuals living with Parkinson’s Disease and their families by launching a Parkinson’s Disease Registry, introducing Dance for PD®, and expanding support groups

For the West Virginia Chronic Kidney Disease Prevention Program
